Preparing for Pregnancy: The 12-Week Runway That Changes Everything
Pregnancy outcomes improve dramatically when you start preparing 3 months ahead. A practical pre-conception checklist from an obstetrician.
Most women plan weddings for six months but pregnancies for six days. The good news: the 12 weeks before you conceive shape the pregnancy more than any single month of it. Here is the runway checklist I give to every woman thinking of starting a family.
Month 3 to 2 — the foundation
- Start folic acid now. 400 micrograms daily, at least a month before trying, cuts neural-tube defect risk dramatically.
- Check vaccines. Rubella and hepatitis B status; tetanus booster if due. Done before pregnancy — live vaccines are avoided while pregnant.
- Book a pre-conception visit. Blood pressure, anaemia screen, thyroid, blood sugar and rubella immunity — one blood draw answers it all.
- Sort chronic conditions. Asthma, diabetes, BP, thyroid: optimise them now so pregnancy starts in a calm body.
Month 2 to 1 — the reset
- Aim for a healthy weight band. Not "skinny" — medically healthy. Both underweight and overweight affect ovulation and outcomes.
- Alcohol: stop or near-stop. Heavy drinking is the clear risk; the safest known target is zero.
- Smoking: stop. This is the single biggest lifestyle lever for fertility and baby health.
- Caffeine under 200mg/day (about one small coffee).
- Start tracking cycles — not for "perfect" timing, but because your doctor will need the pattern.
The last month — the practical
- Dental check-up. Gum disease is linked to preterm birth; fix it before.
- Kitchen safety: store food properly — listeria risk starts now.
- Luggage note: if you travel, check malaria-risk zones — certain antimalarials aren't pregnancy-safe.
- Know your employer's maternity policy so stress doesn't start there.
The "stress is making it worse" truth
Stress doesn't stop conception as dramatically as folklore claims, but cortisol isn't a helper either. So: start the checklist early, and let the process be slower than the checklist. Couples who breathe conceive more.
